Once a letter request has been submitted, please allow at least 3 business days to process your request. If you need a letter that is not among the examples listed, please see an International Education & Exchange Advisor during advising hours.

Address Update Form	Online	7/27/11	Online form for updating your address information. All F-1 and J-1 international students are required by U.S. law to notify their school of any change in U.S. address or home country address. For Students who work on Campus — Please print and submit the Student Action Request (SPAR) Form to Payroll Office at CP-700
Emergency Contact Information	Online	7/27/11	In case of an emergency, please complete and submit this form. In the event that the emergency occurs after office hours which are Monday through Friday 8:00 a.m - 5:00 p.m., please contact the campus police/public safety: 657-278-2515. If it is a life-threatening emergency, please call the police 911.
Financial Disclosure Form	PDF	3/15/11	This form is required when applying for International Student scholarship. Click here for more information regarding available scholarships to students.
Form R	Online	7/27/11	The completion of this form is required when requesting expedited processing of a Form I-20 for the purpose of renewing a California Driver's License, or passport; travel; or emergency situations such as lost or stolen I-20. This form is for use by CSUF students only.
Health Insurance Refund Request Form	PDF	5/13/11	Please read the form carefully before submitting it. Deadlines for submitting refund request are as follows: Fall semester: August 31 Spring semester: February 28
International Student Financial Affidavit Form	PDF	7/22/11	The completion of this form is required in order for the International Education and Exchange office to process a Form I-20 (F-1) or DS2019 (J-1). Please do the following: Complete this form online before printing Sign and date this form Submit the completed print-out together with a copy of passport to the International Education & Exchange Office, UH244.
International Student Scholarship Application	PDF	7/7/09	This form is required when applying for International Student scholarship. Click here for more information regarding scholarships available to students.
Non-Enrollment Notification Form	Online	7/27/11	If you are not planning to enroll in any classes during this current semester, please complete and submit this form.
OPT Employer Information	Online	7/27/11	All F-1 student on approved Optional Practical Training must use this online form to report to their school about their OPT employment. Please complete this online form and submit your OPT Employer Information
Program Extension Request	PDF	5/3/11	Please check with the IEE Office for the deadline for submitting this form. When submitting this form, you must include an updated International Student Financial Statement & Affidavit form with supporting financial documents dated within the last six months.
Reduced Course Load Form	PDF	6/1/09	Refer to the reduced course load guidelines for more information.
Transfer-In to Cal State Fullerton Form	PDF	6/9/09	After you have been admitted to Cal State Fullerton, please print and complete this form. You must contact your Advisor at your previous school and give this form to your previous school, together with your acceptance letter from Cal State Fullerton. Refer to the Immigration section of the Internatisonal Education and Exchange website for more information on Transferring to Cal State Fullerton.
Transfer-Out to Another School Request	PDF	8/5/11	After you have been admitted to another school, complete this form and bring it with your acceptance letter from your new school to your meeting with the CSUF International Student Advisor. Refer to the Immigration section of the International Education and Exchange website for more information on Transferring to Another School.
Travel Signature Request Form	PDF	7/27/11	Please complete and submit this form to request a travel signature. F-1 and J-1 students must obtain signature on their SEVIS documents before leaving the U.S.
